Summative Assignment Guidance
Summative Assignment: Students are required to submit a 3000 word change proposal in a
report format.
The change proposal should identify a problem/challenge in practice that could be
addressed and developed to enhance the service user experience. Critically analyse how
the change could be implemented with specific reference to change management,
leadership and quality improvement models.

This is an Interprofessional change proposal so it is important to remember that
changes in one agency or one part of the system can affect demand and delivery in
another. This interdependency can create risks if not properly considered but also
brings with it the possibility of agencies working together to better meet the needs
of the populations they serve, and achieve wider system efficiencies.
You are actively encouraged to consider service users as a member of the
Interprofessional team.
Keep your change proposal targeted to a local demand. But remember that you will
need to read a variety of audit reports and/or government policy and/or local health
reports to ascertain why change is currently needed within this aspect of health and
social care. For examples, please look at local practice case studies published by NICE

It is very important to consider how you would go about implementing the change at
local level. This should be underpinned with a critical analysis analyse of appropriate
change management, leadership and quality improvement models.
Your report should be a means of informing and persuading people as well as
initiating change.
It should be written in the third person and not include personal pronouns such as
“I” or “we”.
It should be written in sections with numbered sub-headings. Start numbering with
the introduction.

Abstract
Approx. 50 words that distil the essence of the report.
Introduction
A broad overview of the change proposal and what you are seeking to achieve
Justification for change which you have identified from practice.
Briefly describe the expected nature of the change management project.
Background
Provide background information and explain why the change is needed at local level
and how this relates to national findings. Remember to consider relevant legislation
and policy to support your argument as well as your observations from practice.
Critically debate how the change may affect members of the interprofessional team
and how the concept of co–production will be actioned.
Implementing the change (You may want to present this part of the report in sections)
Relate the proposed change with specific reference to change management, leadership and
a quality improvement models. This section should include a critical analysis of systematic
and formal frameworks (models) supporting Quality Improvement (QI), such as:
Lean
Six Sigma
PDSA Model for improvement
Change management models
Remember that you will need to:
Justify and explain your chosen change management model/framework
Justify and explain your leadership approach
Discuss and define the impact on other interprofessional team members,
organizations and within the principles of co-production
Discuss how change will be evaluated, for example what measures will be used
Discuss and evaluate how the change proposal will enhance quality of care for
service users
Suggest ways of rolling out the proposed change

Conclusion
Summarise the key components within the report. Tie together, or integrate the various
issues, research, etc., covered in the main sections of the report the paper, and make
comments upon the meaning of all of it. This includes noting any implications resulting from
your discussion of the topic, as well as recommendations, forecasting future trends, and the
need for further research
